Making an appointment for a diagnostic evaluation |

STEP 1 Call to inquire about an evaluation and schedule an appointment Our appointment coordinator can answer any questions you may have about the evaluation process, discuss concerns related to your child’s development, and assist you in compiling your child’s intake packet. STEP 2 Complete the registration/intake packet You can access the packet by clicking the “intake” button below. Completion of all paperwork prior to your child’s appointment is essential. A completed packet will consist of registration forms, history, questionnaires, and copies of any related evaluations from other professionals. This information will be assembled into a report for review by each member of our multi-disciplinary team. There is a one-time, non-refundable registration processing fee of $50.00. Please be sure to read all of the intake paperwork and provide signatures where required. STEP 3 Return completed registration/intake packet to C.A.R.E Your child’s appointment date will not be confirmed until the intake packet and registration information is completed and received. You can return your information by STEP 4 Schedule appointments Once your packet has been received, our team will begin to review the information. Your child will be scheduled to see each member of our multi-disciplinary evaluation team. Several visits will be scheduled within two weeks of each other. After your child has been evaluated by each professional, the team will meet to generate recommendations, set goals, and compile your child’s evaluation “road map”. STEP 5 Receipt of “Road Map” Your child’s evaluations and team recommendations will be gathered into a full, comprehensive report that we refer to as your “road map”. As a team, we are committed to helping parents to navigate the maze of early intervention, access local resources, and identify appropriate interventions to get your child’s therapeutic program started. The “road map” will help you to understand your child’s development, identify research-based treatment options, find qualified therapists, and get the training you may need to feel confident in addressing your child’s needs. STEP 6 Follow-up appointment We will schedule an appointment within six months of your initial visits to follow-up on your child’s development and ensure that you and your family are supported. At this time, we will make any additional recommendations and answer additional questions. Download registration information/intake packet here |
